Columbia University Waiver and Release Form

I acknowledge that participating in the Columbia University 5K Run/Walk event is a potentially hazardous activity. I am voluntarily entering, and assume all risks associated with, participating in the event, including, but not limited to, falls; contact with other participants, spectators or others; the effect of the weather, including heat and/or humidity; traffic and the conditions of the course; and all such risks being known and appreciated by me.
I agree that by entering and participating in the 5K Run/Walk event that I am healthy, medically able and properly trained, and agree that it is my responsibility to consult with a physician prior to my participation in the 5K Run/Walk event to determine if I am medically able to participate in the 5K Run/Walk event.
As a condition of my participation in the 5K Run/Walk event at a location of my choosing, I agree and acknowledge that neither Columbia University, nor its trustees, officers, agents or employees (the "University"), shall have any liability to me or to my heirs or legal representatives as a result of any loss, injury or damage (including death) incurred or suffered by me in connection with my participation. I hereby expressly waive all rights, claims, causes of action and the like, of any nature whatsoever, which I or my heirs or legal representatives may have against the University in connection with my participation in the 5K Run/Walk event at the location of my choosing.
By submitting my signature below, I hereby authorize Columbia University and those acting pursuant to its authority to use any information or photographs or similar medium that I submit of my participation in the 5K Run/Walk (collectively, “Recordings”), and exhibit or distribute such Recordings in whole or in part without restrictions or limitation for any purpose (including Columbia University websites and social media accounts) which the University, and those acting pursuant to its authority, deem appropriate.
I understand that the University owns all rights in the Recordings and is free to distribute the Recordings as it determines without obligation to me.
This waiver and release shall be binding on me, my heirs, executors and assigns.
